If you're talking about the latest anime ending, that wasn't drawn by Kishi. He doesn't do the anime (though he does watch and occasionally advise it). Based on manga panels alone, Sakura has never had cleavage :p

This does actually have the potential to happen. Tsunade was apparently flat-chested for a little while, according to Jiraiya. "I still can't believe flat-chested Tsunade grew such massive jugs and became the fifth Hokage!"
